免疫球蛋白联合美罗培南对新生儿败血症患儿疗效观察及对细胞因子和免疫功能影响

摘要:
目的:探讨新生儿败血症患儿应用免疫球蛋白联合美罗培南治疗疗效及对细胞因子和免疫功能影响。方法:选择嘉兴市妇幼保健院于2022年6月~2024年6月新生儿败血症98例,依据随机数字表法分为观察组49例与对照组49例。对照组静滴美罗培南,观察组患儿在美罗培南基础上静滴免疫球蛋白。两组均于治疗7d评价。比较两组败血症患儿治疗疗效和症状消失时间;治疗前后PCT和CRP水平,细胞因子和免疫功能变化。结果:观察组败血症患儿总有效率高于对照组(P<0.05)。观察组败血症患儿体温正常、皮肤颜色正常和据奶改善时间短于对照组(P<0.05)。两组治疗后败血症患儿PCT和CRP水平低于治疗前(P<0.05);观察组治疗后败血症患儿PCT和CRP水平低于对照组(P<0.05)。两组治疗后败血症患儿IL-6和TNF-α水平低于治疗前(P<0.05);观察组治疗后败血症患儿IL-6和TNF-α水平低于对照组(P<0.05)。两组治疗后败血症患儿IgA、IgG和IgM水平高于治疗前(P<0.05);观察组治疗后败血症患儿IgA、IgG和IgM水平高于对照组(P<0.05)。结论:免疫球蛋白联合美罗培南治疗新生儿败血症疗效显著,且可减轻患儿细胞炎症反应,及提高患儿免疫功能,值得临床借鉴。

Abstract:
Objective: To explore the therapeutic effect of immunoglobulin combined with meropenem on neonatal sepsis and its impact on cytokines and immune function. Method: Ninety eight cases of neonatal sepsis from June 2022 to June 2024 at Jiaxing Maternal and Child Health Hospital divided observation group of 49 cases and a control group of 49 cases. The control group received treatment with meropenem, while the observation group received treatment with a combination of meropenem and immunoglobulin. Two groups were evaluated after 7 days of treatment. Compare the treatment efficacy and symptom disappearance time between two groups of children with sepsis; Changes in procalcitonin (PCT) and C-reactive protein (CRP) levels, cytokines, and immune function before and after treatment. Result: The observation group effective rate of sepsis higher than control group (P<0.05). The observation group of children with sepsis normal body temperature, normal skin color and milk improvement time shorter than control group (P<0.05). After treatment, two groups levels of PCT and CRP in two groups of sepsis patients lower than before treatment (P<0.05); observation group levels of PCT and CRP in children with sepsis lower than control group (P<0.05). After treatment, two groups levels of IL-6 and TNF-α in children with sepsis lower than before treatment (P<0.05); observation grouplevels of IL-6 and TNF-α in children with sepsis lower than control group (P<0.05). After treatment, two groups levels of IgA, IgG, and IgM of children with sepsis higher than before treatment (P<0.05); observation group levels of IgA, IgG, and IgM in children with sepsis higher than control group (P<0.05). Conclusion: The use of immunoglobulin combined with meropenem in the treatment of neonatal sepsis is significantly effective, and can reduce cellular inflammatory reactions and improve immune function in children, which is worthy of clinical reference.
