温州地区马尔尼菲篮状菌质谱分型及体外药物敏感性研究

摘要:
目的 探讨温州地区不同质谱分型的马尔尼菲篮状菌(TM)对9种常用抗真菌药物的敏感性。 方法 基于基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)技术对收集的35株TM菌株进行同源性聚类分析,Sensititre YeastOne显色药敏板检测9种抗真菌药物对不同质谱分型TM菌株的最低抑菌浓度(MIC)。 结果 MALDI-TOF MS聚类分析将TM菌株分成两大簇,两组分型在药物敏感性与患者呼吸系统表现上存在差异。药敏结果显示泊沙康唑的MIC最低,阿尼芬净、米卡芬净、卡泊芬净和氟康唑的MIC处于较高水平,泊沙康唑、伊曲康唑、伏立康唑、5-氟胞嘧啶和两性霉素B的MIC处于较低水平。 结论 温州地区TM菌株可被MALDI-TOF MS同源性聚类分析分为两种不同分型;体外药敏试验结果显示TM敏感性较高的抗真菌药物有泊沙康唑、伊曲康唑、伏立康唑、5-氟胞嘧啶和两性霉素B,敏感性较低的药物有氟康唑、阿尼芬净、米卡芬净与卡泊芬净。

MALDI-TOF MS-based clustering and antifungal susceptibility of Talaromyces marneffei strains in the Wenzhou region

Abstract:
Objective To investigate the susceptibility of different mass spectrometric types of Talaromyces marneffei (TM) in the Wenzhou region to nine common antifungal drugs. Methods A total of 35 TM strains were collected and subjected to homology clustering analysis based on matrix-assisted laser desorption/ionization-time-of-flight mass spectrometry (MALDI-TOF MS) technology. The minimum inhibitory concentrations (MICs) of the nine antifungal agents against the different mass spectrometric types of TM strains were determined using the Sensititre YeastOne colorimetric susceptibility testing plates. Results The MALDI-TOF MS clustering analysis divided the TM strains into two clusters, revealing differences in drug susceptibility and respiratory manifestations in patients between the two groups. The susceptibility results indicated that posaconazole had the lowest MIC, while anidulafungin, micafungin, caspofungin, and fluconazole had relatively high MICs. The MICs for posaconazole, itraconazole, voriconazole, 5-fluorocytosine, and amphotericin B were comparatively lower. Conclusions TM strains in the Wenzhou region can be classified into two distinct types using MALDI-TOF MS homology clustering analysis. In vitro susceptibility test results show that the antifungal drugs with higher sensitivity against TM include posaconazole, itraconazole, voriconazole, 5-fluorocytosine, and amphotericin B, while fluconazole, anidulafungin, micafungin, and caspofungin exhibit lower sensitivity.
