目的 探讨短时体能测试(SPPB)评分对接受腹部大手术患者术后严重并发症风险的预测价值。方法 回顾性分析2022年6月至2023年11月在本院接受腹部大手术的358名患者,术前均接受SPPB评估,并根据Clavien-Dindo分级系统对术后30天内的严重并发症进行分级。采用Logistic回归分析SPPB得分与术后并发症的关系,并通过LASSO回归构建列线图,采用ROC曲线下面积(AUC)、校准曲线、精确率 - 召回率曲线(PRC)进行模型验证。结果 39名患者(10.9%)发生术后严重并发症,SPPB得分与并发症风险显著相关(P < 0.001)。基于SPPB得分构建的列线图可以预测术后严重并发症风险。结论 SPPB是一种有效的术前评估工具,能够预测腹部大手术后的严重并发症风险,为临床决策提供支持。 |

Objective To investigate the predictive value of the Short Physical Performance Battery (SPPB) score for the risk of severe postoperative complications in patients undergoing major abdominal surgery.Methods A retrospective analysis was conducted on 358 patients who underwent major abdominal surgery at our institution between June 2022 and November 2023. All patients received SPPB assessments prior to surgery, and severe postoperative complications within 30 days were classified according to the Clavien-Dindo grading system. Logistic regression analysis was used to evaluate the relationship between SPPB scores and postoperative complications, and a nomogram was constructed using LASSO regression. The model was validated using the area under the receiver operating characteristic curve (AUC), calibration curves, and precision-recall curves (PRC).Results A total of 39 patients (10.9%) experienced severe postoperative complications, and SPPB scores were significantly associated with the risk of complications (P < 0.001). The nomogram based on SPPB scores effectively predicted the risk of severe postoperative complications.Conclusion The SPPB is an effective preoperative assessment tool that can predict the risk of severe complications following major abdominal surgery, providing valuable support for clinical decision-making. |
