超声造影技术对睾丸肿瘤样病变的诊断价值

摘要:
【摘要】目的 研究超声造影技术在睾丸肿瘤样病变的诊断价值及不同类型睾丸病变的超声造影特点。方法 回顾性分析2018年1月至2024年4月在浙江省人民医院确诊的31例原发性睾丸肿瘤(肿瘤病变23例、肿瘤样病变8例)的灰阶超声及彩色多普勒超声特点和病灶超声造影特性,比较不同类型睾丸病变的一般超声特征,着重分析超声造影技术对不同类型病变的诊断价值。结果 在病变形态、边界、回声以及血流方面,睾丸肿瘤组与肿瘤样病变组差异均无统计学意义(P>0.05);而在超声造影增强异质性、分支血管、周边组织增强征象方面差异均具有统计学意义(P<0.05)。灰阶超声与彩色多普勒超声及超声造影对睾丸不同类型病变的诊断敏感性、特异性、准确率分别为(71.3%vs90.9%)、(63.6%vs95.2%)、(83.6%vs97.5%),两种方法对于睾丸肿瘤样病变组间的诊断价值比较无统计学意义。 结论 超声造影技术在不同类型睾丸肿瘤样病变的诊断中有重要的价值,且在鉴别良、恶性肿瘤性病变和非肿瘤性病变敏感性、特异性相对于常规超声检查均较高。此外,超声造影技术还能够敏感地反映睾丸病变中的微血管灌注情况,为睾丸病变提供一个新的诊断思路,具有一定的临床意义。

【Abstract】Objective To study the diagnostic value of contrast-enhanced ultrasound in testicular tumor-like lesions and the characteristics of contrast-enhanced ultrasound in different types of testicular lesions. Methods A retrospective analysis was performed for the grayscale ultrasound, color Doppler ultrasound and contrast-enhanced ultrasound of lesions in 31 cases of primary testicular tumors (23 cases and 8 cases of tumor-like lesions) diagnosed in Zhejiang Provincial People's Hospital from January 2018 to April 2024. Results There were no significant differences in lesion morphology, borders, echo and blood flow between the testicular tumor group and the tumor-like lesion group (P>0.05), but there were significant differences in contrast-enhanced ultrasound heterogeneity, branching vessels and peripheral tissue enhancement signs (P<0.05). The sensitivity, specificity and accuracy of grayscale ultrasound, color Doppler ultrasound and contrast-enhanced ultrasound for the diagnosis of different types of testicular lesions were (88.5% vs. 100%), (64.3% vs. 92.9%), and (80.0% vs. 97.5%), respectively, and there was no statistical significance for the comparison of the diagnostic value of the two methods among the testicular tumor-like lesions. Conclusion Contrast-enhanced ultrasound imaging has important value in the diagnosis of different types of testicular tumor-like lesions, and its sensitivity and specificity are higher than those of conventional ultrasonography in distinguishing benign and malignant neoplastic lesions from non-neoplastic lesions. In addition, contrast-enhanced ultrasound imaging can also sensitively reflect the microvascular perfusion in testicular lesions, providing a new diagnostic idea for testicular lesions, which has certain clinical significance.
