| 【摘要】 目的 探讨人表皮生长因子受体2(HER2)低表达的非特殊型浸润性乳腺癌(IBC-NST)中激素受体(HR)表达状态与腋窝淋巴结(ALN)转移的相关性。方法 回顾性收集 2018 年 1 月至 2023年 12 月在绍兴市妇幼保健院确诊为HER2低表达IBC-NST的91例患者的临床病理资料,分析不同的免疫组织化学(IHC)判读方法下雌激素受体(ER)、孕激素受体(PR)、雄激素受体(AR)表达状态与ALN转移间的关联性。结果 本研究中ER、PR、AR阳性率分别为90.1%(82/91)、85.1%(78/91)、82.4%(75/91)。AR在ALN转移组与无转移组的表达状态存在显著差异,比较有统计学意义(P<0.05)。IHC阳性细胞占比结合染色强度的判读标准中,当ER和(或)PR处于高表达时,AR同步高表达的ALN转移率显著低于AR中低表达或阴性组,组间比较有统计学意义(P<0.05)。结论 结合染色强度和阳性占比的IHC评分法相较于传统二分法,有助于细化分组并体现组间差异;HR间的同步表达状态可作为评估HER2低表达IBC-NST中ALN转移的预测因子。 |

| 【 Abstract 】 Objective To analyze the correlation between hormone receptor (HR) expression and axillary lymph node (ALN) metastasis in invasive breast cacinoma of no special type (IBC-NST) with low expression of human epidermal growth factor receptor 2 (HER2). Methods The clinicalpathologic data of 91 IBC-NST patients with low expression of HER2 admitted to Shaoxing Maternity and Child Health Care Hospital from January 2018 to December 2023 were retrospectively analyzed. The correlation between the expression of estrogen receptor (ER), progesterone receptor (PR), androgen receptor (AR) and ALN metastasis under different immunohistochemical (IHC) interpretation methods was analyzed. Results The positive rates of ER, PR and AR were 90.1% (82/91), 85.1% (78/91) and 82.4% (75/91) in this study, respectively. There was a significant difference in the expression of AR between the ALN metastasis group and the ALN non-metastasis group (P<0.05). In the interpretation criteria of proportion of HC positive cells combined with staining intensity, when ER or PR were in high expression, the ALN metastasis rate of AR synchronous high expression group was significantly lower than that of AR medium and low expression or negative group (P<0.05). Conclusion The IHC scoring method combined with staining intensity and positive proportion helps to refine the grouping and reflect the differences between groups. The synchronous expression status of HR can be used as a predictor to evaluate ALN metastasis in IBC-NST with low expression of HER2. |
