居家主动循环呼吸技术对COPD稳定期患者肺功能及多维健康状况的影响

摘要:
目的 探讨居家主动循环呼吸技术(ACBT)对慢性阻塞性肺疾病(COPD)稳定期患者肺功能及多维健康状况的影响,为COPD患者的长期管理提供科学依据和实践指导。 方法 采用前瞻性随机对照研究设计,纳入2023年6月至2024年6月在温州医科大学附属温州中心医院呼吸与危重症医学科门诊的COPD稳定期患者60例,随机分为对照组和居家ACBT组,每组30例。所有患者均接受常规治疗,居家ACBT组在此基础上增加居家ACBT训练,干预期为2个月,随访期为1个月。主要观察指标包括患者的依从性,肺功能、改良英国医学研究委员会呼吸困难指数(mMRC)、6分钟内步行距离(6MWT)、医院焦虑抑郁量表(HADS)及COPD评估测试量表(CAT)。 结果 居家ACBT组依从性与对照组相似。在治疗1个月后,肺功能指标显著改善(P<0.05),HADS评分和CAT评分也明显优于对照组(P<0.05)。治疗2个月后,居家ACBT组的mMRC评分较对照组显著改善(P<0.05),而6MWT则在治疗3个月后显示出显著差异(P<0.05)。此外,居家ACBT组的康复效果可持续至停止干预后1个月。 结论 居家ACBT训练能够有效改善COPD稳定期患者的肺功能、心理状态、呼吸困难症状和运动耐力,且效果可持续至干预结束后1个月。居家ACBT具有较高的依从性和可行性,为COPD患者提供了一种灵活且有效的康复模式。

Effect of Home-Based ACBT on Pulmonary Function and Multidimensional Health Status in Stable COPD Patients

Abstract:
Objective To investigate the impact of home-based active cycle of breathing techniques (ACBT) on pulmonary function and multidimensional health status in patients with stable chronic obstructive pulmonary disease (COPD) and to provide scientific evidence and practical guidance for the long-term management of COPD. Methods A prospective randomized controlled trial was conducted, enrolling 60 stable COPD patients from the outpatient department of Respiratory and Critical Care Medicine at Wenzhou Central Hospital affiliated to Wenzhou Medical University between June 2023 and June 2024. Participants were randomly assigned to either a control group or a home-based ACBT group, with 30 patients in each group. All patients received standard treatment, while the ACBT group additionally underwent home-based ACBT training for 2 months, followed by a 1-month follow-up period. Primary outcome measures included patient compliance, pulmonary function, modified Medical Research Council Dyspnoea Scale (mMRC), six-minute walk test (6MWT), Hospital Anxiety and Depression Scale (HADS), and COPD Assessment Test (CAT). Results The compliance of the ACBT group was similar to that of the control group. After 1 month of intervention, the ACBT group showed significant improvements in lung function parameters (P<0.05), as well as better HADS and CAT scores compared to the control group (P<0.05). At 2 months, the ACBT group demonstrated significantly greater improvements in mMRC scores(P<0.05), while the 6MWT showed a significant difference at 3 months(P<0.05). Furthermore, the rehabilitation effects in the ACBT group persisted for 1 month after the intervention ended. Conclusion Home-based ACBT rehabilitation effectively improves lung function, psychological status, dyspnea symptoms, and exercise tolerance in patients with stable COPD, with sustained benefits for at least 1 month post-intervention. ACBT demonstrates high compliance and feasibility, offering a flexible and effective rehabilitation model for COPD patients.
