| 子宫肌瘤作为一种常见的良性盆腔肿瘤,对女性的生活质量及心理产生影响。目前,肌瘤剔除术虽为常见治疗手段,但其术后复发风险较高。而在药物治疗领域,可供选择的药物种类及疗效存在一定局限性。本文旨在通过深入总结临床经验,以选择性孕激素受体调节剂——醋酸乌利司他为研究对象,探索改善子宫肌瘤临床症状的有效途径。过去数年间,大量实验研究表明,醋酸乌利司他能够通过 TGF-β/Samd2/3 通路、MMPs 通路、TNF-α 通路,以及促进细胞凋亡、增强细胞自噬等机制,对子宫肌瘤细胞的增殖过程产生抑制作用,从而展现出醋酸乌利司他在子宫平滑肌瘤临床实际治疗中的巨大潜力。然而,对于这些作用的下游通路及其具体运行机制,目前尚缺乏深入且系统的探讨与研究。基于此,本文将综合现有研究成果,针对当前研究的不足之处,对未来该领域的研究方向及潜在收益提出几点浅见。 |

| Uterine fibroids are among the most common benign pelvic tumors in women and significantly affect both quality of life and psychological health. Although myomectomy is widely used as a treatment option, it carries a substantial risk of disease recurrence following surgery. In the field of drug treatment, the types of available drugs and their therapeutic effects have certain limitations. This article aims to explore effective ways to improve the clinical symptoms of uterine fibroids by deeply summarizing clinical experience and taking the selective progesterone receptor modulator - ulipristal acetate as the research object. Accumulating evidence from recent experiments demonstrates that ulipristal acetate inhibits uterine fibroid cell proliferation via a diverse set of mechanisms. These range from modulating the TGF-β/Smad2/3, MMPs, and TNF-α pathways to directly promoting programmed cell death and autophagy. Together, these findings pave the way for its clinical application in managing uterine leiomyoma. Nevertheless, there remains an absence of comprehensive and structured exploration and analysis regarding the subsequent pathways and detailed mechanisms underlying these effects. In light of this, the present article aims to consolidate existing research outcomes, address the deficiencies in current studies, and offer several potential suggestions for future research avenues and advantages in this area. |
