对比依托泊苷+阿糖胞苷+聚乙二醇化重组人粒细胞集落刺激因子与传统化疗方案在干细胞动员中的疗效分析

摘要:
目的 比较依托泊苷联合阿糖胞苷+聚乙二醇化重组人粒细胞集落刺激因子(简称EAP方案)与传统化疗+重组人粒细胞集落刺激因子(G-CSF)对淋巴瘤和骨髓瘤自体造血干细胞的动员效果及安全性。方法 回顾性分析2018年 10月~2025年5月浙江省金华市中心医院使用EAP方案与传统化疗+G-CSF方案组动员后进行自体干细胞移植的34例淋巴瘤及多发性骨髓瘤患者的临床资料。比较两种方案在采集天数、采集CD34+细胞数、采集过程中血液学毒性、感染风险以及对自体干细胞移植细胞植入的影响。结果 EAP组达干细胞采集的时间短于传统化疗组,分别为13天和15天;采集过程中白细胞最低值、粒细胞最低值,EAP组均高于传统化疗组,粒细胞缺乏时间在EAP组中更短;传统化疗组发生感染事件情况更高,住院周期及总费用更高。两组干细胞动员后对自体干细胞移植的细胞植入和输血负担方面无差异。结论 EAP动员方案达干细胞采集时间短,血液学不良反应低,感染风险低,缩短住院天数,减轻住院费用,相比较传统化疗联合G-CSF,更为推荐。

Abstract:
Objective To compare the mobilization efficacy and safety of the etoposide - cytarabine - pegfilgrastim(EAP regimen) with conventional chemotherapy plus G-CSF for autologous hematopoietic stem cell transplantation in lymphoma and myeloma patients. Methods We retrospectively analyzed the clinical data of 34 lymphoma and multiple myeloma patients who underwent autologous stem cell transplantation after being mobilized with either the EAP regimen or conventional chemotherapy plus G - CSF at Jinhua Central Hospital, Zhejiang Province, from October 2018 to May 2025.The two regimens were compared in terms of the number of collection days, CD34+ cell count, hematologic toxicity during collection, infection risk, and the impact on engraftment after autologous stem cell transplantation. Results The EAP group took less time (13 days) for stem cell collection than the conventional chemotherapy group (15 days). The EAP group had higher nadir white blood cell and granulocyte counts during collection and a shorter duration of granulocytopenia. The conventional chemotherapy group had a higher incidence of infection events, a longer hospitalization time, and higher total costs. No differences were observed between the two groups in terms of engraftment or transfusion burden. Conclusion This study shows that the EAP mobilization regimen achieves faster stem cell collection, lower hematologic adverse effects, reduced infection risk, a shorter hospitalization time, and lower costs compared to conventional chemotherapy group.Based on the current results, the EAP regimen shows potential as a preferred option.
