抽线式皮内缝合联合重组人表皮生长因子在急诊面部创伤患者的疗效分析

摘要:
【目的】:探讨抽线式皮内缝合联合重组人表皮生长因子局部应用在急性面部创伤患者的临床疗效及美学效果。【方法】:选取2022年6月至2024年12月于我院急诊就诊的面部创伤患者80例,先按照随机数字表法分为观察组(40例)和对照组(40例),观察组采用抽线式皮内缝合技术联合重组人表皮生长因子局部应用,对照组仅采用标准连续皮内缝合技术。比较两组患者伤口愈合情况、术后第3天、7天视觉模拟评分(Visual Analog Scale,VAS)、术后第2周、4周、12周温哥华疤痕评价量表(Vancouver Scar Scale,VSS)评分、术后第3周、12周瘢痕评价量表(Patient and Observer Scar Assessment Scale, POSAS)评分及患者术后并发症、满意度调查情况。【结果】1、观察组术后2周的愈合优良率为97.5%,较对照组85%的提升具有明显统计学显著性(χ2=3.91,P<0.05);2、两组术前VAS基线水平相当(t=-1.45,P>0.05),但术后第3天、7天VAS评分观察组显著低于对照组P<0.01);3、两组术后VSS、POSAS评分随时间推移呈下降趋势,且观察组明显优于对照组 (P<0.01);4、观察组术后3月内并发症发生率仅为2.5%,显著低于对照组的15%(χ2=3.92,P<0.05),观察组满意程度达95%,明显优于对照组的80%(χ2=4.11,P<0.05)。【结论】:抽线式皮内缝合联合重组人表皮生长因子的综合治疗方案在改善面部外伤患者术后疤痕、促进伤口愈合、缓解术后疼痛及提高满意度等方面均优于单纯连续皮内缝合方式,具有良好的临床应用价值。

Efficacy Analysis of drawstring Intradermal Suture Combined with Recombinant Human Epidermal Growth Factor in Patients with Emergency Facial Trauma

Abstract:
Objective To explore the clinical efficacy and aesthetic effect of drawstring intradermal suture combined with local application of recombinant human epidermal growth factor in patients with acute facial trauma.Methods Selected eighty patients with facial trauma who visited the emergency department of our hospital from June 2022 to December 2024,and divided into an observation group (40 cases) and a control group (40 cases) according to the random number table method. The observation group adopted the drawstring intradermal suture technique combined with the local application of recombinant human epidermal growth factor, while the control group adopted the standard continuous intradermal suture technique.Compare the wound healing status, Visual Analog Scale (VAS) scores on postoperative days 3 and 7, Vancouver Scar Scale (VSS) scores at postoperative weeks 2, 4, and 12,Patient and Observer Scar Assessment Scale (POSAS) scores at postoperative weeks 3 and 12, postoperative complications, and patient satisfaction survey results between the two groups of patients.Results 1.The excellent and good healing rate of the observation group at 2 weeks after surgery was 97.5%, which was significantly higher than 85% of the control group (χ2=3.91, P<0.05).2.The baseline levels of VAS were comparable between the two groups before surgery (t=-1.45, P>0.05), but the VAS scores of the observation group were significantly lower than those of the control group on the 3rd and 7th days after surgery (P<0.01).3.The VSS and POSAS scores of the two groups showed a downward trend over time, and the observation group was significantly better than the control group (P<0.01);4.The complication rate of the observation group within 3 months after surgery was only 2.5%, significantly lower than 15% of the control group (χ2=3.92,P<0.05). The satisfaction rate of the observation group reached 95%, significantly better than 80% of the control group (χ2=4.11, P<0.05).Conclusion The comprehensive treatment protocol combining retrievable intradermal suture with recombinant human epidermal growth factor has shown superior efficacy in improving postoperative scars, promoting wound healing, alleviating postoperative pain, and enhancing patient satisfaction in patients with facial trauma, compared to the simple continuous intradermal suture alone, and thus holds favorable clinical application value.
