基于单中心的浙江省女性人乳头瘤病毒感染的空间分布热力图分析

摘要:
[目的] 本研究旨在系统分析浙江省女性人乳头瘤病毒(human papillomavirus HPV)感染的空间分布特征,识别高危型HPV基因型的区域聚集性特征,为疫苗接种和宫颈癌筛查措施提供科学的参考意见。[方法]基于浙江省肿瘤医院体检中心12899名研究对象的数据,采用HPV分型试剂盒检测21种HPV基因型,通过地理信息系统绘制感染热力图,并运用非参数Kruskal-Wallis检验分析不同区域和基因型感染率的显著差异。 [结果]浙江省女性的总体HPV感染率为20.20%,高危型基因型以HPV16(5.17%)、HPV52(3.45%)和HPV58(2.77%)为主。空间分布显示,丽水市和衢州市的感染率显著高于其他地区,而杭州市和嘉兴市相对较低。HPV基因型的空间分布具有显著差异,例如,HPV16型主要集中在绍兴市和舟山市,HPV52型和HPV58型则在丽水市和衢州市更为常见。感染率在30-39岁最低,50岁以上显著升高且随年龄增加进行性上升。 [结论] 本研究揭示了浙江省HPV感染的空间分布异质性及主要高危基因型的分布规律。研究结果认为在浙江省,特别是在丽水市和衢州市等高风险地区,推广HPV九价疫苗可能是更好的选择,同时应该更关注50岁以上女性的宫颈癌筛查。

Spatial Heatmap Analysis of human papillomavirus Infection Among Women in Zhejiang Province: Insights from a Single-Center Study

Abstract:
[Objective] This study aims to systematically analyze the spatial distribution of human papillomavirus (HPV) infections among women in Zhejiang Province, identify regional clustering patterns of high-risk HPV genotypes, and provide scientific evidence to optimize vaccination strategies and screening programs. [Methods] Data were collected from 12,899 participants at Zhejiang Cancer Hospital. HPV genotyping was performed for 21 HPV genotypes using specialized kits. Geographic information systems were used to create heatmaps of infection rates, while the Kruskal-Wallis non-parametric test was applied to evaluate significant differences in infection rates across regions and genotypes. [Results] The overall HPV infection rate among women in Zhejiang Province was 20.20%, with HPV16 (5.17%), HPV52 (3.45%), and HPV58 (2.77%) being the most prevalent high-risk genotypes. Spatial analysis revealed significantly higher infection rates in Lishui and Quzhou compared to Hangzhou and Jiaxing. Distinct spatial patterns were observed for HPV genotypes, with HPV16 predominantly found in Shaoxing and Zhoushan, and HPV52 and HPV58 more prevalent in Lishui and Quzhou. The infection rate was lowest among women aged 30–39 years and increased significantly after age 50, showing a progressive increase with age. [Conclusion] This study reveals spatial heterogeneity in HPV infection distribution across Zhejiang Province and identifies the dominant high-risk genotypes. The findings suggest that promoting the nonavalent HPV vaccine may be particularly beneficial in high-risk areas such as Lishui and Quzhou. Additionally, enhanced cervical cancer screening efforts should be prioritized for women over 50 years of age.
