目的探讨体外膜肺氧合(ECMO)对严重心力衰竭患者血浆心钠肽(ANP)、脑钠肽(BNP)、N末端脑钠肽(NT-proBNP)水平的影响。方法选取2013年1月至2015年3月入住ICU的严重心力衰竭患者30例作为研究对象(观察组),同时选取30例健康体检人群作为对照组,观察两组血浆ANP、BNP、NT-proBNP水平的差异及使用ECMO支持治疗后血浆ANP、BNP、NT-proBNP水平变化情况。结果治疗前严重心力衰竭患者血浆ANP、BNP、NT-proBNP的水平分别为(1214.8±388.6)、(258.6±148.6)、(429.5±386.6)pg/ml,明显高于健康人群组(均P<0.05);患者在ECMO前后各时点ANP、BNP、NT-proBNP水平的总体差异均有统计学意义。其中ANP水平在ECMO运行1h后明显升高,之后逐渐降低,并与开始前30min相似;BNP水平在ECMO运行后逐渐升高,至3h时已较开始前30min明显升高,至36h时才有所下降;NT-proBNP水平在ECMO运行24h时才明显升高,36h时未有明显下降。经ECMO治疗52~72(62.4±9.4)h后,22例患者病情稳定,均顺利撤除ECMO装置,顺利出院,占73.33%;死亡8例,占26.67%。结论ECMO支持治疗对严重心力衰竭患者血浆ANP、BNP、NT-proBNP水平具有明显的影响。 |

Objective To investigate the effect of extracorporeal membrane oxygenation (ECMO) on plasma levels of brain natriuretic peptide (BNP), atrial natriuretic peptide (ANP) and N-terminal brain natriuretic peptide (NT-pro BNP) in patients with severe heart failure. Methods Thirty patients with severe congestive heart failure were enrolled from January 2013 to March 2015, and 30 healthy subjects served as control group. Plasma levels of ANP, BNP and NT-pro BNP were measured before and during ECMO treatment. Results Plasma ANP, BNP and NT-proBNP in patients with severe heart failure was (1 214.8±388.6) pg/ml, (258.6 ±148.6) pg/ml and (429.5 ±386.6) pg/ml before ECMO treatment, which were significantly higher than those in healthy
controls (P<0.05). Plasma levels of ANP, BNP and NT-proBNP were gradually decreased with the running of ECMO and there were significant differences at different time points (P<0.05). All 30 patients were successfully withdrawn from the ECMO device, among whom 22 (73.33%) were discharged and 8 died (26.67%). Conclusion ECMO support treatment may significantly affect plasma ANP, BNP and NT-pro BNP levels in patients with severe heart failure. |
