目的 探讨小于胎龄儿(SGA)胎盘血、脐带血淋巴细胞中 Clock、Bmal1 基因表达及与胰岛素、胰岛素样因子 -1 (IGF-1)的相关性。 方法 选择 2013 年 1—12 月新生儿 67 例,其中 SGA 31 例,适于胎龄新生儿(AGA)36 例;采用酶化学发光法 测定脐带血、胎盘血胰岛素、IGF-1,采用竞争性 RT-PCR 法测定胎盘血、脐带血淋巴细胞中 Clock 、Bmal1 基因表达水平。 结果 SGA 组出生体重、BMI、身高、胎盘血胰岛素和 IGF-1 水平、脐带血胰岛素和 IGF-1 水平均低于 AGA 组(均 P<0.05);胎盘血、脐带血 胰岛素水平,脐带血 IGF-1 水平与出生体重均存在相关性(均 P<0.01)。SGA 组胎盘血、脐带血淋巴细胞中 Bmal1 基因表达水平均高 于 AGA 组(均 P<0.01);而 Clock 基因表达水平差异均无统计学意义(均 P >0.05)。脐带血、胎盘血淋巴细胞中 Clock、Bmal1 基因表 达水平与出生体重、胰岛素、IGF-1 水平均无相关性(均 P >0.05)。 结论 SGA 血清胰岛素和 IGF-1 水平较低,脐带血、胎盘血淋巴 细胞中 Bmal1 基因表达水平较高;新生儿生理节奏性的发展受母体生物钟节奏的影响。 |

Objective To investigate the expression of Clock, Bmal1 mRNA and the levels of insulin, Insulin-like growth factor type-1 (IGF-1) in umbilical and placental blood small for gestational age (SGA) newborns. Methods Sixty-seven cases of newborns delivered in Zhuji Maternal and Child Health Care Hospital from January to December 2013 were enrolled in the study, including 31 SGA infants and 36 appropriate for gestational age(AGA) infants. The levels of insulin and IGF-1 in cord and placental blood were measured by enzymology bioluminescence; the expressions of Clock and Bmal1 mRNA in cord and placental lymphocytes were tested by competitive RT-PCR in both groups. Results The weight and BMI in SGA group were significantly lower than those in the AGA group (P<0.05). The levels of Insulin, IGF-1 and expression of in SGA were significantly lower than those in AGA group (P<0.05). The expression of Bmal1 mRNA in SGA group was significantly higher than that in AGA group (P<0.05), while there was no significant difference in expression of Clock mRNA between two groups (P>0.05). The expression of Bmal1 mRNA was not correlated with the weight of newborn, insulin and IGF-1 levels in umbilical and placental blood (P>0.05).Conclusion SGA newborns have lower insulin, IGF-1 levels and higher expression of Bmal1 mRNA. The maternal Clock rhythm influences the neonatal circadian rhythm. |
