目的探索游离脂肪酸(FFA)水平与糖尿病患者肾脏功能的关系。方法回顾186例2型糖尿病患者的临床资料,根据所有糖尿病患者肾小球滤过率(eGFR,采用改良肾病饮食改善公式估算)将患者分为3组,A组[eGFR>90ml/(min·1.73m2)]58例,B组[60ml/(min·1.73m2)≤eGFR<90ml/(min·1.73m2)]74例,C组[eGFR<60ml/(min·1.73m2)]54例。观察3组患者FFA水平与患者eGFR的关系。结果A组患者FFA水平为(296.38±121.57)μmol/L,B组为(351.73±140.58)滋mol/L,C组为(421.38±178.52)μmol/L,根据Spearman相关性检验,患者eGFR与FFA水平呈负相关(r=-0.552,P<0.05);logistic多因素回归分析显示,FFA增高为糖尿病患者eGFR降低的独立危险因素。结论FFA的增高是糖尿病患者肾功能损害的危险因素之一,在糖尿病患者肾功能损害中起着重要作用,在治疗糖尿病时不仅要控制血糖,降低尿蛋白定量,还要积极降低患者血脂,延缓糖尿病患者肾功能损害的速度,改善预后。 |

Relationship between free fatty acid levels and renal function in diabetic patients |

Abstract: |
Objective To examine the relationship between serum free fatty acid (FFA) levels and renal function in patients with diabetes mellitus. Methods The clinical data of 186 patients with type 2 diabetes (T2DM) admitted in our hospital from May 2013 to November 2016 were retrospectively analyzed. According to the glomerular filtration rate (eGFR) patients were divided into three groups: group A (eGFR >90ml/min/1.73m2, n=58), group B (60ml/min/1.73m2 ≤eGFR <90ml/min/1.73m2, n=74) and group C (eGFR<60ml/min/1.73m2, n=54). The relationship between FFA levels and renal function was analyzed. Results The FFA levels in groups A, B and C were (296.38 ±121.57) 滋mol/L, (351.73 ±140.58) 滋mol/L and (421.38 ±178.52)滋mol/L, respectively. Spearman correlation showed that eGFR was negatively correlated with the FFA level (r=-0.552, P <0.05). Multivariate logistic regression analysis showed that elevated FFA was an independent risk factor for eGFR reduction in diabetic patients. Conclusion Elevated FFA is one of the independent risk factors for eGFR reduction in diabetic patients. |
