		     | 目的探讨结直肠癌组织中miR-21表达与临床病理因素及预后的关系。方法取59例结直肠癌组织标本及癌旁正常黏膜组织标本,采用茎环逆转录实时荧光定量PCR方法检测结直肠癌组织及癌旁正常黏膜组织中miR-21的相对表达量,并分析癌组织中miR-21表达与临床病理因素的关系。用Kaplan-meier法计算生存率,采用log-rank检验、Cox比例风险回归模型分析结直肠癌预后影响因素。结果结直肠癌组织中miR-21的相对表达量明显高于其癌旁正常黏膜组织(P<0.05)。结直肠癌组织miR-21表达水平与肿瘤直径、淋巴结转移、临床分期有关(均P<0.05);与性别、远处转移、分化程度、浸润深度和肿瘤位置无关(均P>0.05)。远处转移、分化程度、miR-21表达水平是影响患者预后的独立危险因素(均P<0.05),其RR值及95%CI分别为5.053(1.395~5.787)、3.998(1.395~18.305)、3.837(1.266~11.631)。结论miR-21与结直肠癌患者预后和疾病进展密切相关。 |

                | Objective     To   investigate   the   expression   of   miR-21   in   colorectal   cancer   and   its   association   with clinicopathological  characteristics  and  the  prognosis  of  patients.     Methods     The  expression  of  miR-21  in  59  specimens  of colorectal  cancer  and  matched  non-tumor  adjacent  tissue  specimens  was  examined  with  stem-loop  real-time  RT-PCR.  The relationship   between   the   expression   of   miR-21   and   clinicopathologic   features   of   colorectal   carcinoma   was   analyzed. Kaplan-Meier survival curves, log-rank test and multivariate Cox regression were used to analyze the factors related to prognosis of colorectal cancer.    Results    The expression level of miR-21 was significantly higher in colorectal cancer tissue than that in the
matched normal colorectal mucosa  (2.698±1.973 vs. 0.653±0.213, P<0.05). The expression of miR-21 was positively correlated
with tumor size, lymph node metastasis and clinical stage (P<0.05), but not with sex, distant metastasis, cell differentiation, depth of tumor infiltration and location (P >0.05). Kaplan-Meier univariate analysis showed that distant metastasis, cell differentiation, depth of tumor infiltration and miR-21 was the prognostic factor for colorectal cancer (P <0.05). The Cox regression analysis showed that distant metastasis, cell differentiation and miR-21 was an independent prognostic risk factor of colorectal cancer(P<
0.05), RR value and 95%CI was 5.053 (1.395-5.787), 3.998 (1.395-18.305) and 3.837 (1.266-11.631). Conclusion The results indicate that miR-21 serves as a molecular marker for disease progression and prognosis of colorectal cancer. |
