CCL5在膀胱癌中的表达及临床意义

摘要:
目的检测趋化因子配体5(CCL5)在膀胱癌中的表达情况,探讨CCL5在膀胱癌中的临床意义。方法选择136例膀胱移行细胞癌患者标本和20例正常膀胱组织标本,采用免疫组化SP法检测膀胱癌和正常组织中CCL5的表达,并结合临床资料进行分析。结果正常膀胱组织中CCL5的表达阳性率明显低于膀胱癌组织,膀胱癌组织中随着病理分级和分期的增加,CCL5的表达阳性率逐渐增高。CCL5在低级别与高级别膀胱癌中高表达阳性率分别为27.3%和55.6%,差异有统计学意义(P=0.001)。肿瘤分期Ta~T1和T2~T4CCL5高表达阳性率分别为38.3%和57.1%,差异有统计学意义(P=0.041)。本组患者随访7~117个月,Kaplan-Merier分析CCL5高表达阳性率患者的术后生存时间明显降低(P<0.01);多因素研究分析,CCL5的表达是影响患者术后总生存期的独立预后因素(P=0.038)。结论CCL5的表达阳性率在膀胱癌组织中明显高于正常组织,检测CCL5的表达有助于膀胱癌的诊断及预后评估。

Abstract:
Objective To investigate the expression of CCL5 in tumor tissue of patients with bladder cancer and its clinicopathological significance. Methods The expression of CCL5 protein in 136 specimens of bladder cancer and 20 specimens of normal bladder tissues was detected by SP immunohistochemtry, its correlations with clinicopathologic features of patients was analyzed. Results The expression of CCL5 was higher in tumor tissues than that in normal bladder tissues. The positive rate of CCL5 in low grade bladder cancer was lower than that in high grade bladder cancer (27.3% vs. 55.6%, P=0.01); the expression rate in non-muscle-invasive bladder cancer was lower than that in muscle-invasive bladder cancer (38.3% vs 57.1%, P=0.041). The patients were followed up for 7-117 months. Kaplan-Meier survival analysis demonstrated that CCL5 expression was significantly associated with shorter overall survival (P <0.01). Multivariate analysis further demonstrated that CCL5 was an independent prognostic factor for patients with bladder cancer. Conclusion The expression of CCL5 protein is highly in bladder cancer, the detection of the expression of CCL5 protein can be used for diagnosis and prognosis of bladder cancer.
