目的探讨microRNA-26a在原发性肝癌(HCC)患者中的表达及临床意义。方法选取HCC、慢性乙型肝炎(CH)患者各28例。HCC患者留取癌组织、癌旁组织、血浆标本,CH患者留取血浆标本,采用RT-PCR法检测标本microRNA-26a的表达水平。比较HCC患者与CH患者microRNA-26a表达水平;分析HCC患者癌组织、癌旁组织microRNA-26a表达水平的相关性;比较血浆microRNA-26a与血清甲胎蛋白(AFP)诊断HCC的灵敏度、特异度;以血浆microRNA-26a水平最佳临界值(2.873)为界,将HCC患者分为microRNA-26a高表达组和低表达组,比较两组患者术后1年生存率。结果HCC患者血浆microRNA-26a表达水平低于CH患者(P<0.05)。HCC患者癌组织microRNA-26a表达水平低于癌旁组织(P<0.05)。HCC患者癌组织、癌旁组织microRNA-26a表达水平呈正相关(r=0.574,P<0.038)。血浆microRNA-26a诊断HCC的灵敏度较血清AFP强(85.8%vs57.8%,P<0.05),但特异度较血清AFP弱(65.2%vs80.52%,P<0.05)。microRNA-26a高表达组患者术后1年生存率高于低表达组(60.25%vs45.77%,P<0.05)。结论HCC患者microRNA-26a表达水平较低,microRNA-26a可作为早期诊断HCC的生物标志物,也可作为HCC患者预后评估指标。 |

Objective To investigate the expression of microRNA-26a (mRNA-26a) in primary hepatocellular carcinoma (HCC) and its clinical significance. Methods The expression of mRNA-26a were detected with RT-PCR in plasma, cancer tissue and pericancerous liver tissue samples from 28 patients with HCC and plasma samples from 28 patients with chronic hepatitis B (CHB). The clinical significance of mRNA-26a expression in HCC was analyzed. Results The expression level of plasma microRNA-26a in HCC patients was lower than that in CHB patients (P<0.05). The expression level of microRNA-26a in cancer tissues of HCC patients was lower than that of pericancerous liver tissues (P<0.05). There was a positive correlation between the expression of microRNA-26a in cancer tissues and pericancerous liver tissues (r=0.574, P<0.05). The sensitivity of plasma microRNA-26a in diagnosis of HCC was higher than that in serum AFP (85.8% vs 57.8%, P<0.05), but the specificity was
lower than that of serum AFP (65.2% vs 80.52%, P<0.05). The 1-year survival rate of patients with high expression of mRNA-26a was higher than that of low expression group (60.25% vs 45.77%, P<0.05). Conclusion The expression level of mRNA-26a in HCC tissues is down-regulated, it may be used as a molecular marker for early diagnosis of HCC. |
