目的探讨超声心动图检查预测布洛芬治疗新生儿动脉导管未闭(PDA)效果的价值。方法选取经超声心动图检查发现动脉导管未闭合的新生儿155例,记录动脉导管的直径、血流速度,布洛芬常规治疗后复查超声心动图,分别比较动脉导管直径、血流速度与闭合率的关系。结果出生24h内动脉导管的血流速度与7d后动脉导管的闭合率呈正相关(r=0.219,P<0.01),而出生24h内动脉导管的直径与7d后动脉导管闭合率无关(r=-0.152,P>0.05)。动脉导管血流速度≥2.0m/s较<2.0m/s闭合率显著升高(字2=7.622,P<0.01),在早产儿组中差异亦具有统计学意义(字2=6.652,P<0.01)。结论对于动脉导管未闭合的新生儿(早产儿),通过超声心动图测定的动脉导管血流速度预测布洛芬常规治疗后动脉导管的闭合率有一定的应用价值,血流速度为2.0m/s是较好的分界点,≥2.0m/s时可以选择布洛芬常规方案治疗;<2.0m/s时应考虑增加布洛芬剂量或延长用药时间。 |

Objective To predict the efficacy of ibuprofen for patent ductus arteriosus (PDA) in neonatals by echocardiography. Methods One hundred and fifty five newborns with PDA received regular treatment of ibuprofen. The diameter and blood flow velocity of ductus arteriosus were examined by echocardiography before and after treatment. Results The blood flow velocity within 24h after birth was positively correlated with closure rate of ductus arteriosus 7 days later (r=0.219, P<0.01), and when the blood flow velocity was >2.0m/s the closure rate elevated significantly (字2=7.622, P <0.01); but the diameter of ductus arteriosus was not significantly correlated with the closure rate (r=-0.152, P >0.05). Conclusion The blood flow velocity of ductus arteriosus detected by echocardiography can predict the closure of patent ductus arteriosus in newborns
under the treatment of ibuprofen, particularly when the velocity is above 2.0m/s. |
