鲍温样丘疹病皮损中朗格汉斯细胞与MCP-1的表达及意义研究

摘要:
目的检测鲍温样丘疹病(BP)皮损中朗格汉斯细胞(LC)的分布与数量变化及单核细胞趋化蛋白-1(MCP-1)的表达,探讨它们在BP发病机制中的作用。方法采用免疫组化SP法检测36例BP患者皮损组织中CD1a+LC及MCP-1的表达情况,以皮损旁正常皮肤组织作为对照;并对CD1a+LC密度与MCP-1表达强度进行相关性分析。结果CD1a+LC主要见于表皮棘层的树突状细胞,MCP-1蛋白主要表达于表皮角质形成细胞。与皮损旁正常皮肤组织相比,BP皮损组织中CD1a+LC密度降低,分布不规则,胞体树突减少;MCP-1染色变弱,表达强度降低。BP皮损组织及皮损旁正常皮肤组织中CD1a+LC密度与MCP-1表达强度均呈正相关(r=0.372、0.384,均P<0.05)。结论BP皮损表皮组织中LC密度下降,形态呈现变性改变,推测可能与皮损局部趋化LC并促进其分化成熟的趋化因子MCP-1表达降低有关,进而影响皮损局部LC的抗原递呈功能。

Distribution of Langerhans cells and expression of MCP-1 in skin lesions of Bowenoid papulosis and their clinical significance

Abstract:
Objective To investigate the distribution of Langerhans cells (LCs) and the expression of monocyte chemoattractant protein-1 (MCP-1) in skin lesions of Bowenoid papulosis (BP) and their clinical significance. Methods The expression of CD1a (marker of LC) and MCP-1 in skin lesions and adjacent normal skin tissue of 36 patients with BP were detected with immunohistochemical staining. The correlation between the density of CD1a+ LCs and MCP-1 expression was analyzed. Results CD1a+ LCs was mainly expressed in dendritic cells in the spinous layer of the epidermis. MCP-1 was mainly expressed in the cytoplasm of epidermal keratinocytes. In epidermal BP lesions, CD1a+ LCs density was lower, and these cells exhibited an abnormal distribution and shorter protrusions, and MCP-1 staining was weaker and the semi-quantitative score of MCP-1 expression was lower than those in the adjacent normal epidermal tissue. The density of CD1a+ LCs was positively correlated with MCP-1 expression in BP lesions and the adjacent normal skin tissue. Conclusion The decreased LC density and degenerated LC morphology in the epidermis may be related to the down-regulation of MCP-1 expression in BP lesions, ultimately leading to immune suppression.
